Form 5500-EZ is an annual informational return used by sponsors of our Single(k) plans to report the plan’s financial and operational status. It is required by the IRS, Department of Labor (DOL), and Pension Benefit Guaranty Corporation (PBGC) to ensure compliance with federal regulations and maintain transparency about plan administration and funding.
Who Must File Form 5500-EZ?
You are required to file Form 5500-EZ for your Single(k) plan if:
- The plan’s total assets exceeded $250,000 as of December 31st, or
- The plan was terminated during the year (regardless of the account balance)
If neither condition applies, you are not required to file a 5500-EZ for that plan year. If you are a Single(k) Plus plan sponsor, Ubiquity tracks this requirement and will automatically prepare the Form 5500-EZ for you as needed.
How Can I File Form 5500-EZ?
You have two options:

Submission Deadlines (for 2024 Form 5500-EZ Filings)
To ensure timely preparation, submit your request to Ubiquity by the following deadlines:
- June 1, 2025 – If you are not filing an extension
- September 1, 2025 – If you have filed a Form 5558 extension
Once your form is ready, you’ll receive a notification in early to mid-June (or early to mid-September if extended) that your 5500-EZ is available for electronic signature.

Form 5500-EZ Delinquent Filers
If you missed a filing for a prior year, you may still be able to file and reduce penalties through available penalty relief programs.
